Parental Release for Interviewing, Photographing, Videotaping/Audio-Recording, and Web Postings of Students

I recognize that, as part of the educational process, there may be times when DAFA, or someone properly authorized by DAFA, such as media representatives, may want to interview, photograph, and/or videotape a student. Consequently, I grant permission to DAFA and to anyone properly authorized by DAFA, to interview, photograph, audio- record and/or videotape my student during regular school hours on school grounds and/or at student activities. This grant of permission mean that, in addition to other appropriate uses (which include, but are not limited to, inclusion in or DAFA publications, promotional materials, advertisements, presentations, and programs) information obtained from the student and the student’s likeness and name may be used in conjunction with or by any medium, including print, electronic, radio, and television. This form also grants permission for posting student images on the DAFA website ( I waive any cause of action I may have, or that the student may have, against DAFA pertaining to the reproduction, publication, and/or use of information obtained from the student and/or the student’s name or likeness. I acknowledge that I may revoke this Release at any time during the year by mailing or delivering a written notice of revocation to the Executive Director, Gladys West, at Digital Arts for Autism, P.O. Box 234, Windermere, FL 34786.

Digital Arts for Autism (DAFA) POLICY(S)


The MONTHLY cost of the DIGITAL MEDIA TRAINING PROGRAM is $875.00 (USD) and is part of a comprehensive four course training program.

CUSTOMER PAYS (3) EQUAL PAYMENTS for a total of $2,625.00 (USD) per course. authorized to a credit card on file. 

Customer understands and acknowledges this is a four course training program and TOTAL cost of training program is $10,500.00 (USD) for Digital Media Training from Digital Arts for Autism.

* First equal payment due upon enrollment | Second & Third payments due on the 1st of each consecutive month continuously throughout length of four course training program. No Refunds, Exchanges, or Credits once enrolled. All pricing based on continuous enrollment in the training program. 

What personal data we collect and why we collect it


When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.


If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
